Our Team

  • The Administrative Services Team consists of a Manager, three Senior Analysts and six Administrative Analysts. CSLLC’s staff provides excellent customer service, and we offer plan implementation and flexible benefit educational services.

Document Preparation

Consultation and Design

A Combined Services LLC Representative will setup a consultation to review an employer's current plan designs. The information gathered is used to produce customized plan documents and summary plan descriptions. To ensure the documents meet IRS and/or ERISA guidelines and to maintain legal compliance, CSLLC utilizes the services of leading providers of benefit plan documents to third party administrators.

We offer documentation preparation for a one-time fee. No subscription required.

Premium Conversion Plan / Premium Only Plans

A Premium Conversion Plan, also known as a Premium Only Plan (POP) is a pre-tax plan defined via Section 125 of the IRS Code. If an employer is deducting insurance premiums from employee salaries pre-tax, a premium conversion plan must be in place. The plan indicates that employees pay for their share of group health, life, dental and disability insurance premiums with pre-tax dollars. (If the disability premiums are taken pre-tax, the benefit then becomes taxable).

    ERISA Wrap Documents

    A Wrap SPD and Wrap Plan Document are designed to wrap around existing certificates of insurance and benefit plan booklets (health, dental, vision, FSA, HRA, prescription drug, accidental death and dismemberment, long-term and short-term disability and group term life plans) to provide the required provisions and information necessary to comply with ERISA. To be compliant, the Wrap SPD and accompanying benefit plan component documents must be distributed to plan participants. We will provide you with the following:

  • Customized Wrap SPD and Wrap Plan Document that provide the required ERISA provisions
  • Distribution guidelines that explain how and when to provide these compliance documents

  • Health Reimbursement Arrangement - Plan Doc Only

    Employers who wish to self-administer a Health Reimbursement Arrangement can do so once they have a HRA Plan Document in place. A Combined Services LLC Representative can go over the plan design options and prepare the plan documents for the employer's HRA.

    Non-Discrimination Testing

    All pre-tax plans are required to complete annual testing to determine whether the plan(s) discriminate in favor of highly compensated employees. If Combined Services LLC performs the testing, we will provide the employer with a template to complete and return securely. We will then complete all seven tests.
